Studies conducted over the last decade have shown that, in the majority of patients with acquired immunodeficiency syndrome, gastrointestinal alterations occur concomitantly with the onset of immunodeficiency. The abnormalities include weight loss, malabsorption of nutrients, reflux esophagitis, and interstitial enteropathy. Severe infection with cytomegalovirus, herpes simplex virus, and candida occurs frequently. Although the pathogenesis of these lesions is not well understood, it appears that a progressive destruction of the gastrointestinal mucosa and alterations in enteric innervation play a role. The combination of gastrointestinal tract disease and malnutrition may contribute to weight loss, which in turn may contribute to a further diminution of immunologic function. In addition, gastrointestinal tract disease has been associated with an increased risk for other morbid complications.
